    DUTIESFORTHISPOSITION INCLUDE,BUTARENOTLIMITED TO:

    Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assume the responsibility for coordination of care. The RN assists with after hour on-call coverage. The Registered Nurse Coordinator acts as direct supervisor to Licensed Practical Nurses, Licensed Vocational Nurses, Certified Nursing Assistants, Homemakers, and other non-licensed personnel while in the patients’ home.

     Professional and clinical services:

    1. Perform new patient intake assessments, including physical, psychological, social, and spiritual parameters. Patients are assigned by the director or other delegated hospice professional.

    2. Develops or assists with the development of a comprehensive plan of care for patients based on the comprehensive assessment.

    3. Makes appropriate referrals to other community agencies as determined by patient/family needs and resources.

    4. Coordinates care with the Hospice team, i.e., Hospice staff, primary physician, volunteers, clergy, and patient/family; responsible for direction and appropriateness of referrals.

    5. Supervises and/or provides direct patient care within the guidelines of registered nursing practice in the state and as defined by the Hospice policies and procedures.

    6. Provides bereavement follow up and/or referral within the guidelines of hospice's bereavement policy.

    7. Shares in 24-hour call coverage as assigned by the Director, or means developed by a mutual team. Call coverage is for all active Hospice patients/families and new referrals received.

    8. Makes referral and assists in the selection of hospice volunteers for specific patients/families. Introduces and supervises the volunteers, when necessary.

    QUALIFICATIONS:

    Under clinical supervision, assists in the delivery of patient care and treatment according to policies and procedures. Provides administrative support and works collaboratively with Physicians, Advanced Practice Clinicians, RNs, LPNs and other department staff to meet the physical, psychosocial, education and spiritual needs of the patient and family.

    Qualifications:

    • Post high school education or vocational training in healthcare preferred. Other combinations of education and clinical setting experience may be considered.
    • Graduate of a CAAHEP or ABHES accredited medical assisting program within eighteen (18) months of hire.
    • Current Health Care Provider CPR preferred or must successfully complete prior to completion of orientation.
    • Medical office experience and medical terminology training preferred. Possess working knowledge of anatomy, physiology, medical terminology, basic laboratory values, and pharmacology preferred.
    • Knowledge and familiarity with medical equipment and instruments, common safety hazards and precautions, patient rights, regulatory standards and aseptic technique and infection control preferred. Knowledge of Health Information Portability and Accountability Act (HIPAA) and confidentiality requirements preferred.
    • Minimum of on   e (1) of the below medical administration requirements. Possess working knowledge of safe pharmaceutical handling requirements and guidelines.

    Job Specific Duties:

    • Prepares room, equipment and supplies as indicated by particular examination or patient’s needs.
    • Cleans and disinfects equipment per assigned area(s) policies and procedures.
    • Assists with appointments, diagnostic tests, treatments, procedures and surgery. May assist with patient registration, collection of co-payments and prior authorizations.
    • Collects patient information from incoming calls and/or patient portal messages for appropriate referrals to nurse, physician and/or advanced practice clinician.
    • Attends required meetings and participates in committees as requested.
    • Receives and prepares patient for examination and treatment per practice procedure and protocol.
    • Obtains and records history, vital signs, and collects laboratory specimens. Accurately charts and documents in medical record per policy and procedure.

    May perform as a scribe and accurately transcribe medical reports. Proofreads all work transcribed to ensure proper patient and physician names, spelling, and punctuation. Applies expert knowledge of medical terminology, anatomy and physiology, English rules of transcription and proofreading of all medical dictation. Maintains up-to-date knowledge of elements of a medical note, new techniques, new medications and new terminology in order to produce quality reports. Researches and follows up on incomplete dictation. Works with providers to assure that all documentation is entered into patient records appropriately.
