We are looking for a bilingual case manager to work in a community and family centered organization. We are looking for someone who has more than 2 years of experience, someone who is willing to work in community and non-profit based work. This is a full time in person position.
Minimum Qualifications & Requirements
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (preferred)
- Bilingual Spanish proficiency (oral and written) required
- Experience working with children and families in community-based settings
- Knowledge of trauma-informed care and strength-based approaches
- Valid California Driver’s License, reliable transportation, and current auto insurance
- Ability to successfully complete background clearances, including Live Scan and Child Abuse Index
Key Responsibilities
1. Family Engagement & Case Management
- Manage referrals from CFS for DR Path II and After Care families
- Participate in required Transition Meetings
- Conduct initial strengths-based family assessments
- Develop individualized Family Service Plans (FSPs) in partnership with families
2. Direct Service & Home Visiting
- Provide direct case management to families experiencing multiple stressors (e.g., domestic violence, substance use, mental health challenges)
- Conduct in-home and in-person visits throughout the community
- Work flexible hours, including evenings and weekends as needed
- Provide hands-on support and coordinated linkage to essential community resources (e.g., housing, employment, mental health services)
3. Documentation & Compliance
- Maintain accurate and timely case records using required DR program data-collection tools
- Document family engagement and service contacts
- Complete case exit plans, closure summaries, and required follow-up contacts
