Medical Care Coordinator
Job Summary: Express Healthcare Staffing is working with a well-established nonprofit providing residential treatment for individuals with substance use disorders, who is seeking a Medical Care Coordinator. This role is ideal for someone who is highly organized, compassionate, and committed to supporting clients through their recovery journey. As a Medical Care Coordinator, you will play a crucial role in ensuring individuals receive necessary medical services, attend appointments, and stay consistent with prescribed medications.
Requirements:
- Assist clients in accessing ongoing services, including medical, dental, vision, housing, financial assistance, and legal advocacy; support clients in obtaining or reactivating disability benefits, Medicaid, and other needed resources.
- Ensure timely medication distribution, support medication compliance, conduct daily medication checks, and document compliance in the EMR system.
- Monitor new or updated medications to ensure appropriate documentation and continuity of care.
- Complete and upload all incidental funds paperwork with accuracy; assist counselors with discharge planning and perform additional duties as assigned.
Qualifications:
- High School Diploma required; Bachelor’s Degree preferred. Medical Assistance Certification preferred.
- Minimum 1 year of Care Coordinator experience; experience working with individuals with substance use disorders preferred.
- Strong communication and problem-solving skills with the ability to work effectively with a diverse population.
- Computer literacy and comfort with EMR systems.
Schedule & Pay:
- Schedule: Monday – Friday, 9:00AM – 5:00PM
- Pay: $18.00 – $20.00 P/H
