Job Title: Case Manager (Evaluation-to-Hire)
Pay: $19.53/hour
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Work Environment: Primarily Remote

We are seeking a compassionate and detail-oriented Case Manager to provide support to clients seeking assistance through CERA and related programs. This role offers meaningful work helping individuals access critical resources while building a career in social services.
Key Responsibilities:
- Provide one-on-one case management and client support services
- Conduct assessments to determine client eligibility for programs and services
- Coordinate internal and external referrals and resources
- Assist clients with documentation, application completion, and follow-up
- Create award vouchers and process payments to landlords and utility providers
- Accurately enter and maintain client data in the agency database
- Participate in outreach efforts to promote available programs and services
Qualifications:
- Strong organizational and communication skills
- Ability to manage multiple client cases and meet deadlines
- Detail-oriented with experience in data entry and documentation
- Professional, empathetic, and client-focused approach
Equipment Provided:
- Company-issued laptop and phone
Additional Requirements:
- Valid driver’s license, current auto insurance, and reliable vehicle required for occasional travel
