Social Services Case Manager
Location: Woodburn, OR
Schedule: Monday–Friday, Full-time
Pay: $27.40/hour 

 


Position Overview:
We are looking for a compassionate, organized professional to support the community. You’ll manage a caseload, assess eligibility, build care plans, and coordinate services in partnership with community agencies and internal teams.

 


Key Responsibilities:
• Interview clients and assess non-financial needs
• Determine eligibility for in-home or community services
• Develop, monitor, and adjust individual care plans
• Coordinate resources such as transportation, equipment, and housing
• Maintain accurate case documentation in internal databases
• Collaborate with internal staff and external providers
• Ensure compliance with privacy and state/federal guidelines

 


Requirements:
• 4 years of experience in social services or financial fields (eligibility, credit, lending, etc.)
• Strong communication, problem-solving, and organizational skills
• Ability to manage multiple priorities in a fast-paced environment
• Comfortable working in office and field-based settings
• Must pass criminal background check
• Valid Oregon driver’s license or reliable transportation

 


Preferred Qualifications:
• Notary Public certification
• Experience supporting vulnerable populations
• Bilingual skills are a plus
• Compassionate, driven, and eager to learn — training provided
