Service Coordinator

Express Employment Professionals is seeking a Service Coordinator for our client which runs retirement communities.
The Service Coordinator plays a crucial role in enhancing the quality of life for elderly tenants within our retirement community. This position involves coordinating services, resources, and support systems to ensure residents' needs are met, fostering a safe, engaging, and supportive living environment that promotes independence and well-being.
Resident Advocacy: Serve as a primary point of contact for residents and their families, addressing concerns, inquiries, and service needs with empathy and compassion.
Service Coordination: Assess individual resident needs and coordinate services, including healthcare, wellness programs, transportation, meal services, and social activities, ensuring access to necessary resources.
Care Plan Development: Collaborate with residents, families, and healthcare professionals to develop personalized care plans tailored to each resident’s physical, emotional, and social needs.
Community Engagement: Organize and facilitate community events, social activities, and educational programs that encourage resident participation and foster a sense of belonging.
Resource Networking: Build and maintain relationships with local service providers, healthcare facilities, and community organizations to connect residents with valuable resources and support.
Documentation and Reporting: Maintain accurate records of services provided, resident interactions, and follow-up actions. Prepare reports for management on service coordination outcomes and resident feedback.
Crisis Intervention: Identify and respond to residents in crisis situations, providing immediate support, and coordinating with emergency services when necessary.
Staff Collaboration: Work closely with facility staff, including nurses, social workers, and administrative personnel, to ensure cohesive service delivery and communication regarding resident needs.
Education: Bachelor’s degree in social work, gerontology, nursing, or a related field is preferred.
Experience: Minimum of 2 years of experience working with elderly populations in a healthcare, social services, or residential setting.
Skills:
Certifications: CPR and First Aid certification preferred.
Knowledge: Understanding of aging processes, common health issues faced by the elderly, and available community resources.
Must have a valid Texas drivers license to be insured for company vehicles
