Job Summary:
The Home Health Scheduler is responsible for coordinating and scheduling patient visits for skilled nursing, therapy (PT, OT, ST), and other clinical services. This role ensures timely initiation of care, continuity of services, and efficient communication between patients, clinicians, and the office. The scheduler serves as a key point of contact to balance patient needs with clinician availability while maintaining compliance with agency policies and regulatory requirements.

 

Key Responsibilities:
• Schedule nursing, therapy, and other clinical visits in accordance with physician orders, agency policy, and patient care needs.
• Ensure timely initiation of care and resumption of care visits per CMS guidelines.
• Communicate promptly with patients and caregivers regarding visit schedules, changes, or delays.
• Maintain and update clinician schedules daily, adjusting for cancellations, availability, and patient needs.
• Collaborate with clinical managers to ensure appropriate staff assignment based on skill set, geography, and patient condition.
• Verify clinician availability and coordinate coverage for urgent or same-day visits.
• Document all scheduling activities accurately in the EMR system.
• Support intake by reviewing new referrals and ensuring smooth scheduling of start-of-care visits.
• Monitor daily and weekly schedules to ensure compliance with frequency and duration orders.
• Serve as liaison between field staff and office for scheduling concerns, problem-solving conflicts in a professional manner.
• Participate in staffing meetings and provide reports on scheduling activity as requested.

 

Qualifications:
• Previous scheduling experience in home health, healthcare, or medical office setting strongly preferred.
• Knowledge of home health regulations and visit requirements is a plus.
• Strong organizational, multitasking, and problem-solving skills.
• Excellent verbal and written communication skills.
• Proficiency in EMR systems and Microsoft Office Suite.
• Ability to work in a fast-paced environment and maintain professionalism under pressure.
• Bilingual preferred
