RN Care Coordinator – BPCI / Orthopedic Focus
📍 Full-Time | On-Site | Mississippi Gulf Coast
We are seeking an experienced RN Care Coordinator (LPN with 10+ years considered) to manage patients through a 90-day episode of care under the BPCI Advanced and other bundled payment programs. This role focuses on orthopedic patients and care coordination across the full continuum of care.
Key Responsibilities:

Manage patients throughout a 90-day bundled care episode

Complete pre-op risk assessments and post-op clinical evaluations

Develop and implement individualized care plans

Coordinate care across acute, post-acute, rehab, SNF, LTAC, home health, and outpatient settings
-
Arrange services including home health, DME, PT, and community resources
-
Provide telephonic and onsite follow-up

Maintain accurate EMR documentation
-
Identify and address care gaps and barriers
-
Ensure HIPAA compliance
Qualifications:
-
Active Mississippi RN license
-
LPN with 10+ years of experience considered
-
2–3 years nursing experience required
-
Orthopedic experience highly preferred

Discharge planning and outpatient care coordination experience preferred

Strong EMR, documentation, communication, and time management skills
-
Ability to work independently in a fast-paced environment
