Job Full Description
Assess client in home to evaluate level of care necessary for future and facilitate necessary course of actions.
Delegate tasks to other case manager
Check in on clients for follow up appointments
Travel is necessary
Must have relevent experience working with the disabled community or a BA in Social Science, Counseling, OT, Speech Therapy, or equivalent combination of eduacation and experience.
