Community Health Care Case Worker (full-time, 17.00/hr, direct hire, comprehensive benefits)

Non-Profit specializing in the care of the elderly community is seeking a Community Health Care Case Worker to join their team. This field-based role will be focused on UF patients returning home or Veteran's Administration patients looking to stay in their home during treatment.

Duties:

• Conducts comprehensive patient assessments for Care Transitions Intervention program to determine appropriateness through a review of patient demographic information, medical information, input from physician or other medical personnel and patient interview.
• Conducts in-home and/or remote client visits, skilled nursing home and/or hospital visits with patient to provide coaching and training to promote patient self-care management, utilizing the appropriate evidenced based coaching tools.
• Conducts telephone visits with the patient to provide coaching and training to promote patient self-care management, utilizing the appropriate tools of the evidence-based models.
• Provides coaching and training to assist patient in recognizing medication discrepancies and reconciling discrepancies with physician or pharmacist.
• Provides training and coaches the patient on how to recognize signs and symptoms of worsening disease and take appropriate measures.
• Encourages and follows-up with patient to schedule and complete appointments with primary care physicians within established timeframes.
• Develops and maintains positive professional relationships with outside service providers.
• Screens patients and coordinates referrals for assistance with community-based services, as appropriate.
• This position requires the employee to travel frequently throughout Florida, as directed. Cost of travel is paid by the traveler and will be reimbursed according to state travel reimbursement policies.

Requirements:

• Bachelor's degree OR equivalent work experience in healthcare/human services
• Professional written and spoken communication skills
• Understanding of Medical terminology
• Customer service mindset
• Accurate typing at or above 40 wpm
