RootedHomes https://rootedhomes.org/ is a non-profit affordable housing developer based in Bend, OR. RootedHomes provides housing opportunities for those who contribute to the fabric of the Central Oregon economy and community. RootedHomes is seeking a Housing Coordinator to support outreach efforts as RootedHomes expands to Crook and Jefferson Counties.
What You Get to Do!
The Housing Coordinator leads housing-focused street outreach and engagement efforts to connect individuals experiencing homelessness to coordinated entry and pathways to permanent housing. This role is responsible for building trust with priority populations, reducing barriers to housing readiness, and supporting participants through the initial stages of the housing continuum. The position operates within a Housing First, trauma-informed, and client-centered framework.
• Conduct regular street outreach at scheduled locations, engaging individuals experiencing homelessness and building relationships through consistent, trust-based interactions.
• Connect participants to coordinated entry systems (e.g., HEAT assessments), housing navigation, and supportive services within 72 hours of engagement.
• Assist participants in obtaining critical documentation (IDs, birth certificates) and accessing services such as record expunction, benefits, and referrals.
• Provide transportation assistance and basic needs support (e.g., survival supplies) to facilitate engagement and housing stability.
• Plan and implement community engagement events in partnership with local and culturally specific organizations to expand outreach reach and effectiveness.
• Maintain accurate and timely client data entry into HMIS within required timeframes (72 hours), in coordination with data/reporting staff.
• Conduct targeted outreach to underserved populations, including BIPOC and first-generation individuals, using culturally responsive and inclusive practices.
• Support outreach communications through social media, print, and community-based strategies to increase awareness and participation.
What You Will Bring…
• Experience in homeless services, street outreach, case management, or a related field.
• Knowledge of Housing First principles, trauma-informed care, and coordinated entry systems.
• Ability to work effectively with diverse and vulnerable populations, including those with lived experience of homelessness.
• Strong organizational skills and ability to manage documentation and data systems (HMIS experience preferred).
• Bilingual skills and/or lived experience with housing instability strongly preferred.
• Excellent written/oral communication skills, with customer-facing priorities.
• Experience with culturally and economically diverse communities.
• Experience working with communities that have been historically excluded and underserved in relation to housing.
• Bilingual.
Working Conditions
• Field-based role requiring travel between counties and outreach in outdoor environments.
• Flexible schedule, including occasional evenings or weekends for outreach and events.
Performance Expectations
• Engage approximately 100 individuals within a 3-month period.
• Refer at least 25% of engaged participants to housing navigation services within 72 hours.
• Ensure accurate, complete, and timely data entry and reporting in compliance with program requirements.
Salary: $32.00/hour
