Job Title: Wraparound Coordinator
City and State: Crystal Lake, IL
Pay Rate: $26.14
Schedule/Shift: 8-5pm
Employment Type: Direct Hire
Summary:
The Wraparound Coordinator partners with families and referral sources to develop and lead a Child and Family Team using a strengths-based, trauma-informed approach. This role ensures implementation of individualized care plans aligned with the National Wraparound Initiative Model, supporting families in achieving their identified goals.
Responsibilities:
- Facilitate Child and Family Team development, meetings, and ongoing collaboration.
- Lead the creation, implementation, and evaluation of individualized, strengths-based service plans.
- Identify barriers, develop solutions, and ensure fidelity to the Wraparound model.
- Support families in engaging natural supports and community resources.
- Complete required documentation and ensure compliance with confidentiality and program standards.
Qualifications:
- Bachelor’s degree in Social Work or related Human Services field.
- Experience in case management, social services, or child welfare preferred.
- Strong communication, organization, and problem-solving skills.
- Ability to work flexible hours, including evenings and occasional weekends.
- Valid driver’s license, insurance, and ability to pass background screenings.
