Job Full Description
Job Duties:
- Manage client cases
- Assess needs
- Set goals
- Document progress
- Connect clients with resources
- Follow up on action plans
- Monitor risk and stability
- Make sure that clients receive respectful and timely support
- Keep records accurate and up to date
- Customer service
- Use computers and computer programs
- Answer phone
Requirements:
- High School Diploma or equivalent is required
- Additional education in human services, social work, family services, behavioral health, education or related field is helpful
- Experience helping individuals or families through customer service, non profit services, community work, healthcare education, case support or similar setting
- Able to communicate clearly and efficiently
- Able to actively listen and follow verbal and written instructions
- Organized
- Good with critical thinking and problem - solving
- Comfortable working on and with computers and computer programs
- Ability to sit, stand, walk, bend, reach
- Able to answer multi-line phone
- Excellent customer service and phone etiquette skills
Work Schedule: 8am to 4:45pm Monday - Friday
