HomeBase Case Manager

Department: HomeBase & Secure Jobs Initiative
Reports To: Manager of Housing Counseling & Financial Services
Location: Worcester County Area
Employment Type: Temp to Perm

Pay Rate:  $25.00–$27.00


Position Summary

The HomeBase Case Manager assists individuals and families facing homelessness in achieving stable housing and sustainable employment. This role provides comprehensive re-housing and stabilization case management services to participants in the HomeBase program, focusing on housing search support, financial stability, employment readiness, and connection to community resources.

The Case Manager is responsible for conducting client intakes and assessments, developing individualized housing stabilization plans, coordinating supportive services, and maintaining accurate documentation and reporting.


Key Responsibilities

  • Conduct comprehensive client assessments, gathering financial, employment, housing, education, and health information to develop individualized re-housing and stabilization plans.

  • Provide exceptional customer service to clients, staff, and community members.

  • Respond to calls and inquiries in a timely manner, including walk-ins.

  • Provide case management support to emergency assistance clients and advocate during housing search efforts.

  • Develop supportive subsidy plans and coordinate furniture and moving assistance.

  • Conduct client visits and assist families with housing search and placement opportunities.

  • Acquire required documentation and ensure rental units meet program health and safety standards.

  • Develop and coordinate support systems that promote independent living, self-sufficiency, and family stabilization.

  • Make appropriate referrals to social service agencies, educational institutions, medical providers, and employment resources.

  • Monitor and verify services on a weekly/monthly basis to evaluate effectiveness.

  • Maintain accurate and up-to-date records in web-based systems (E2E and ETO) and virtual files.

  • Complete monthly reporting for the Finance Department and funders.

  • Manage and maintain an active caseload with proper documentation.

  • Perform other duties as assigned by the supervisor.

 


Qualifications & Experience

  • Excellent verbal and written communication skills.

  • Ability to effectively communicate with individuals from diverse cultural and socioeconomic backgrounds.

  • Strong organizational skills with high attention to detail and the ability to meet deadlines.

  • Ability to prioritize tasks and work collaboratively as part of a team.

  • Demonstrated commitment to exceptional customer service.

  • Strong computer proficiency (MS Word, Excel, Outlook, database systems).

  • Ability to maintain confidentiality and handle sensitive information.

  • Experience working in a fast-paced environment with multiple competing demands.

  • Bilingual (English/Spanish) required.

  • Experience assisting clients with housing search and community resources preferred.


Education Requirements

  • Bachelor’s degree preferred but not required.

  • 1–3 years of experience in human services, housing services, or case management required.


Certifications & Licensure

  • Valid driver’s license and reliable transportation required.

  • Ability to travel throughout Worcester County to meet with clients.

  • Must obtain certification with the Regional Employment Board (R.E.B.) within six (6) months of employment.

  • None


Working Conditions & Physical Requirements

  • Primarily office-based with extended periods of sitting and computer use.

  • Regular movement between sitting and standing; frequent bending and filing.

  • Ability to use standard office equipment and fine motor skills.

  • Ability to speak, listen, and use earpieces/headphones.

  • Occasionally lift 10–15 pounds.

  • Travel within Worcester County required.
