Are you passioniate about childern's rights?   Express Employment is hiring a custodian to manage medical records for a non-profit agency that deals with alleged child abuse.  The ideal candidate will have some background or knowledge in the legal field.   

Position Description Summary

The Custodian of Medical Records is responsible for creating, maintaining, and curating all records produced and maintained by the facility. This includes ensuring the completeness of each child’s medical records and ensuring HIPAA and OARs compliance. They support the provision of direct services by providing administrative support to the Direct Services team and managing databases and statistics.  The Custodian of Medical Records responds to subpoenas and records requests by the court. 

Compensation: $25-$28 an hour DOE

Position Accountabilities and Essential Functions:

80%      Custodian of Medical Records

  • Ensures collection of authorization paperwork from the Assessment team the day of the assessment to ensure completeness of the paperwork.
  • Ensure paperwork is uploaded to CareNet
  • Conduct regular reviews of Medical Records including file counts to ensure the maintenance and preservation of records.
  • Respond to subpoenas for records in a timely and legally sound manner, consulting with legal representation as needed.
  • Ensure that staff respond to subpoenas as appropriate.
  • Respond to request for medical records and ensure staff are maintaining confidentiality protections when distributing records.
  • Document and invoice records releases as appropriate.
  • Develop and maintain tracking systems for all legal documents and responses.
  • Responsible for final report distribution, responding to the receipt of subpoenas and communicating with attorneys.
  • Enter subpoenas in the calendar, spreadsheet and CareNet.
  • Consult with Executive Director and contracted legal representation as needed.
  • Prepare, verify, and distribute medical records that have been subpoenaed, including hand delivery if necessary.
  •  Ensures that all records requests are in accordance with HIPAA and Oregon Administrative Rules (OARs).
  • Updates policies and procedures regarding record distribution and creates a workflow for the upkeep of medical records.

20%    Program Support

  • Client File Management
  •  Direct Service Client File Management for active cases including uploading documents to electronic client management system and preparing billing packets
  •  Consolidate client data from multiple locations on the server into the client management system. Collating documents and cross checking for errors and duplication
  • Respond to urgent requests to convert, consolidate and upload files when a new referral has been made on a client that has been previously served or siblings.
  • Support intake with creating electronic client files during high volume crises
  • Create reports using the CareNet report builder program to pull one-time use data as well as building regular reports to support services and statistics as needed.
  • Work with Direct Service Staff and Program Manager to ensure data fidelity and accurate reporting from staff, including preparing reports analyzing data and identifying gaps in statistics.
  • Act as back-up for cross-trained positions as needed.

Qualifications/Education/Experience:

  • High school diploma/GED required, higher education preferred. Qualified paralegal preferred.
  • Previous experience in grant writing, tracking and reporting a plus
  • Excellent organizational, writing, listening and communication skills
  • Able to follow oral and written instruction
  • Able to perform work independently and as a part of a team
  • Computer skills such as word processing and design software (Microsoft Office Suite, cloud-based software, and database applications)
  • Able to maintain a high level of confidentiality required
  • Strong relationship skills to establish and maintain effective working relationships with community partners, volunteers and clients
  • Excellent customer service skills
  • Creative and flexible under pressure
