Additionally, previous administrative and case management experience is preferred.

The Care Team Specialist will work closely with customers, DSP's (Direct Support Professionals), and county or brokerage-based case workers to support our community with intellectual and developmental disabilities. The goal is to be an ally in helping our customers to become the best version of themselves while having fun in the process!

 

Specific Responsibilities:

  • Maintain positive relationships with our customers and make sure their needs are being met both administratively and by their DSP.
  • Manage our providers and ensure a supportive and positive experience.
  • Respond to emails, calls, texts, and voicemails daily.
  • Conduct monthly customer and provider check-in calls.
  • Build solid customer and provider relationships by handling questions and concerns quickly and professionally
  • Ensure that customer and DSP interactions are properly documented.
  • Maintain customer and provider demographic profiles for accuracy.
  • Assist in training, coaching, and developing our DSP's.
  • Create Quarterly Reports (incl: Approving protocols/action plans with customer/legal guardian, training providers to customer protocols and action plans, and obtaining monthly progress of desired outcomes from customer/legal guardian/provider)
  • Send birthday cards to customers and providers
  • Update and maintain procedures and processes as needed
  • Complete assigned tasks as needed

 

Qualifications:

  • Heart for people
  • Bilingual in Spanish and English
  • 1+ years of office/administrative experience.
  • 1+ years of case management/leadership experience.
  • Proficient with Microsoft 365 (including Outlook, SharePoint, Excel, Word, and Teams)
  • Must have integrity and an eagerness to grow
  • Respect and interest in supporting the I/DD community
  • Attention to detail
  • Pass a motor vehicle report
  • Clear a federal background check
  • Must be at least 18 years old with a high school diploma

Compensation and Schedule:

  • Hourly Rate: $26.25 per hour
  • Monday-Friday: 8:30 AM - 5:00 PM at our Newberg Office (flexibility in hours)
  • Paid Bi-monthly
