Job Full Description
We are seeking a reliable and compassionate Licensed Practical Nurse (LPN) in Taunton, MA to provide part-time in-home check-in visits for patients in the community. This role involves traveling to multiple residences to assess patient well-being, monitor health conditions, and ensure care plans are being followed. Haitian-Creole language skills are highly preferred.
Travel:
- MUST be able to travel from the Cape Cod to Boston areas.
- Compensated for travel expenses
Schedule:
- Part-time, flexible hours
- Travel required between patient homes
Pay: $35.00 per hour
Responsibilities:
- Conduct routine in-home visits to assess patients’ overall health and safety
- Monitor vital signs and report any changes or concerns to the care team
- Ensure patients are following prescribed care plans and medications
- Provide basic nursing care and support as needed
- Maintain accurate and timely documentation of each visit
- Communicate effectively with patients, families, and healthcare providers
- Identify potential risks and report any safety or health concerns
Qualifications:
- Active LPN license in good standing
- Previous experience in home health or patient care preferred
- Strong assessment and communication skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation and valid driver’s license required
